Hospital to Home Service

When you are spending time in hospital, often the thing that keeps you going is the thought of getting back into your own home to recover and get well.  However, if you have received treatment or had an operation, it may not be possible for you to manage independently.

Crossroads provide a Hospital to Home service, where our trained staff support people leaving hospital to transition back to home life.  Their goal is to rehabilitate you and be there to provide help until you are well and able to manage independently and safely.

Support ranges from helping you mobilise, administer medication or with meal preparation.  To access this service, you will need an assessment, so contact Crossroads to find out how we can help you put the arrangements in place.
