Dementia Care

Each member of Crossroads team undertakes comprehensive training and receive qualifications on providing exceptional support for people living with dementia.

Our services are supplemented by specialist Dementia Link Workers, who are trained specifically to support people living with memory-related conditions.

Services for people living with dementia include:

• Help and support to live safely at home
• Activities, clubs and groups with specialist dementia support
• Supporting the Dementia Action Alliance to build a dementia friendly community
• Reminiscence sessions
• Dementia walks
• Memory Café
• Friendship groups
• Training for carers, volunteers, professionals, community groups and partners

We pride ourselves on building strong, trusting relationships with all our service users.  We take a person-centred approach to care, which means building a support plan which is bespoke and individualised.

Crossroads enable people to build confidence and engage with the community, create opportunities for social interaction and help to improve self-esteem.
